This is an example of a Lab Billing Statement. Each row is an invoice LINE item plus the custom fields going back for a couple of months. It can span mutliple pages and the aging and credit card info (not shown) is put on the last page.
The next statement is a one page statement for a single invoice generated from our Meter Billing Program. The different charges shown are lines of the original QuickBooks invoice. It also gets any finance charges and payments and put it on the statement.
